By booking an appointment with Physio & Rehab, you agree to the following terms and conditions.

 

Appointments: Appointments are available by prior booking only. Please ensure a suitable, safe, and private space is available for assessment and treatment within your home.

 

Cancellation Policy: A minimum of 24 hours' notice is required to cancel or rearrange an appointment. Cancellations made with less than 24 hours' notice, or missed appointments, may be charged up to the full appointment fee.Payment: Payment is due at the time of treatment unless otherwise agreed. Accepted payment methods will be confirmed when booking.

 

Assessment and Treatment: Physiotherapy involves assessment, diagnosis, treatment, rehabilitation, and advice. While every effort is made to help you achieve your goals, no guarantee can be given regarding treatment outcomes.Consent: Your informed consent will be obtained before any assessment or treatment is carried out. You are free to withdraw your consent at any time.

 

Medical Information: You agree to provide accurate and complete information regarding your medical history, medications, and any changes to your health that may affect treatment.

 

Confidentiality: Your personal information and clinical records will be handled confidentially and in accordance with UK data protection legislation (UK GDPR and the Data Protection Act 2018). Information will only be shared with your GP or other healthcare professionals with your consent or where required by law.

 

Safety: Treatment may be declined or discontinued if it is considered unsafe, inappropriate, or outside the physiotherapist's professional scope of practice.Professional Standards: All treatment is provided in accordance with the standards of the Chartered Society of Physiotherapy (CSP) and the Health and Care Professions Council (HCPC).

 

Liability: Physio & Rehab accepts no liability for loss or damage to personal belongings during appointments. Clients are responsible for following the advice and exercise programmes provided.
